Hi doctor,

I am a 28-year-old male, and I usually have pleasing morning boners almost two to three days a week, where my penis gets hard and long. But, while masturbating for the last one to two years, it is soft all along and gets a bit harder at the time of ejaculation, and that hardness is way inferior to my morning boner. Six months ago, I tried having sex with my girlfriend. When she was giving me a blow job, it was hard, but as soon as I tried having vaginal sex after removing my penis from her mouth, it went softer very quickly, within 20 to 25 seconds. I find it hard to maintain my erection. Before that, I used to masturbate almost daily for eight to nine years. Then, I reduced it to once a week. After one month of reduced frequency, I tried to have sex with my girlfriend, and the same problem persisted. Then, I did not masturbate for around 20 days and tried having sex, but the same story got repeated. After that, I did not ask my girlfriend. I am worried a lot about how I will do the vaginal sex when it is soft all along. Kindly help me out. I am very tense after this incident about how I will approach my life with this problem. Please suggest a way of treatment without any substantial side effects.

Please tell if you have any medical conditions like diabetes, hypertension, heart problem or restricted blood flow to veins or arteries, etc., which are the leading causes behind erectile dysfunction. Also, excessive use of tobacco or alcohol may lead to this problem. So if you are using these products, then stop their excessive use.

According to your history, everything seems normal. As far as daily masturbation is concerned, that is a normal thing that many young men do. If there is no history of any medical condition, then the most common cause behind it is performance pressure, which occurs in many young men. It can be due to some underlying stress. Stress can be work-related, or it is a fear that you might not be able to perform sex properly with your partner. For your current problem of not able to perform vaginal sex, I advice try doing sex without any worry or guilt that you could not do it properly last time.

I suggest capsule Ashwagandha (Withania somnifera) two capsules two times a day with water after food for two weeks. Take some therapy sessions with a nearby psychologist to gain the lost confidence. Do exercise daily. For example, you can do brisk walks, running, swimming or cycling, etc. If you are overweight, then try to reduce your weight. Eat healthy and fresh food. Add milk, almonds, walnuts, pistachios, eggs (only if you eat), radish, beetroot, citrus fruits, strawberries, and other berries to your food regimen. Try all these things regularly, and you will get better results soon.

I checked my fasting sugar level three to four months ago, and it was expected, around 90 mg/dL. I drink or smoke very rarely for about once in three to four months. But after this fiasco, I am altogether avoiding it. What tests are needed to rule out hypertension, restricted blood flow, and heart problems? I will get these tests done whenever I get time. How much mg of Ashwagandha would be sufficient?

For hypertension, check your blood pressure; if it falls in the normal range, then no need to worry. As far as heart problems and restricted blood flow or any other tests I mentioned, you only need them if you have severe pain. Also, the physician should perform these tests in a hospital. You cannot perform these on your own in a nearby laboratory.

According to your history, I think it is only due to stress and more like a psychological problem related to performance. So be calm next time. I suggest Himalaya's Ashwagandha capsules. Take two capsules two times a day with water after food for two weeks. Each capsule has 250 mg of Ashwagandha in it.

There is one thing I forgot to mention that I have a problem with frequent urination, which starts in the afternoon and goes on till I go to bed at night. During this time, I go to the toilet almost every 10 to 15 minutes. The volume of urine discharged during this period each time is very low. I got all my tests related to kidney, urine including KUB ultrasonography, and everything came out to be expected. Does this cause trouble in maintaining an erection? Also, this problem has become chronic. Please advise an Ayurvedic treatment. During this phase of frequent urination, my penis feels very weak and feels kind of hollow inside.
