Is chest pain with arm heaviness a heart-related issue?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I would like to share the symptoms I am currently experiencing:

  1. I feel pain specifically in the heart area, not around it.

  2. The pain worsens when I sneeze or engage in physical activity.

  3. My left arm sometimes feels tired or heavy.

  4. I rarely use my right arm, and it does not present any issues.

  5. When I exert myself physically, it feels as though the heart muscle itself is hurting.

  6. These symptoms are persistent and tend to worsen with physical effort or strain.

Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I have gone through your query and understand your concern.

Your symptoms, like pain specifically in the heart area that worsens with sneezing or physical effort, along with occasional left arm tiredness or heaviness, do need careful evaluation, especially since they involve the chest and left arm.

Possible considerations include:

  1. Musculoskeletal causes: The fact that pain increases with sneezing or movement may point to a musculoskeletal issue such as costochondritis (inflammation of the chest wall) or a muscle strain.

  2. Cardiac causes: While less common in younger individuals, any chest pain that occurs with physical effort, especially when accompanied by left arm symptoms, must be carefully assessed to rule out heart-related conditions such as angina.

  3. Other causes: Nerve-related issues (like cervical radiculopathy) or referred pain could explain the arm heaviness or fatigue.

What to do next:

  1. See a doctor urgently: Because of the location of the pain and the associated arm symptoms, it is important to get a full clinical evaluation. If you experience additional symptoms like shortness of breath, dizziness, or sweating, seek emergency medical care immediately.

  2. Diagnostic tests: A doctor may order an ECG (electrocardiogram), chest X-ray, and blood tests (such as troponin levels) to evaluate for cardiac causes and other potential issues.

  3. Avoid strenuous activity: Until you have been properly evaluated, it is best to avoid activities that provoke or worsen the symptoms.

Summary:While it might be musculoskeletal, chest pain with these features always requires medical assessment to exclude serious heart problems. Please make an appointment with a healthcare provider promptly or visit an emergency department if symptoms worsen.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.
