Hello doctor,

I am a 36-year-old woman and have two kids (the younger one is four years old). I was diagnosed with PCOS/PMOS eight years ago, but I did not have trouble conceiving, and my periods are normal. However, now I am having a lot of hair fall from the front side, chin hair, regular urinary incontinence, and I cannot lose weight.

I have been overweight since my childhood. My height is 5 feet 1 inch, and my weight is 167.55 pounds. I also had my gallbladder removed due to gallstone issues soon after my second child's birth. My HbA1c was six percent (six months ago). Please suggest how I can improve my health and sort out the problem of urinary incontinence.

Urinary incontinence has been happening for almost one and a half years, but I have been ignorant. The condition aggravates near my periods. I have also had my routine urine and culture tests done, which came out to be normal. Please help.

PCOS (polycystic ovary syndrome)/PMOS (polyendocrine metabolic ovarian syndrome) is a lifestyle-related issue and continues lifelong; at times, it remains in the dormant stage and does not show any symptoms.

Your current symptoms of weight gain or difficulty in losing weight and facial hair growth happen due to excessive testosterone levels in the blood. Decreasing its level and losing weight can solve many PCOS/PMOS-related symptoms.

Urinary incontinence is a different issue, but it can be managed with some Ayurvedic medicines and pelvic muscle exercises. The lower urinary sphincter controls urine flow from the bladder to the urethra.

Due to infection or other issues, the sphincter loses its elasticity and control, which causes the dribbling of urine. Ayurvedic medicines can help strengthen the sphincter muscle.

I suggest the following Investigations to be done:

  • Ultrasonography (USG) of the abdomen and pelvis.

  • Thyroid function test.

  • Serum testosterone.

The treatment plans can be decided after investigations and a video consultation.

For prevention, I suggest doing the following -

  • Practice pelvic floor muscles exercise daily.

  • Avoid sweets, cold, oily, spicy, or junk foods. Also, avoid bakery products.

  • Do anulom vilom pranayam for five minutes daily.

  • For weight loss, I can suggest diet, exercise, as well as medical treatment.
