Can NF1 impact learning and development in kids?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My sister's 6-year-old has been diagnosed with neurofibromatosis type 1. He has 14 café-au-lait spots and freckling under his arms, which the pediatrician confirmed. His development seems delayed; he is not reading yet and has trouble with balance when running. The genetic test showed a spontaneous mutation, as neither parent has NF1. What specialists should he be seeing regularly? The neurofibromas on his torso are starting to appear. Do they keep multiplying throughout his life?

The pediatrician mentioned potential vision and spine issues but did not go into detail. My sister is concerned about his learning difficulties and whether they are connected to NF1. What schooling accommodations should she request? Are there treatments that can slow NF1 progression? His MRI showed two bright spots in his brain that need further investigation. Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I read your query and can understand your concern.

The 6-year-old has been diagnosed with NF1 (neurofibromatosis type 1- a genetic disorder that causes non-cancerous tumors to grow along nerves), confirmed by café-au-lait spots and a genetic test. He has developmental delays, balance difficulties, and early neurofibromas. An MRI showed unidentified bright objects (UBOs) in the brain, which are common in NF1 and usually not harmful. The mutation is spontaneous since neither parent has NF1, which occurs in about 50 % of cases.

Specialists to see regularly:

  1. Neurologist – To monitor brain function, learning issues, and potential seizures.

  2. Ophthalmologist – For yearly checks to detect optic gliomas or vision problems.

  3. Developmental pediatrician – To assess developmental and learning delays.

  4. Geneticist or NF Specialist – For ongoing management of NF1.

  5. Orthopedic doctor – To monitor spine health, especially for scoliosis (a condition where the spine develops an abnormal curve, often appearing S-shaped or C-shaped) or other bone concerns.

Dermatologist – To track the growth of neurofibromas.

Psychologist or learning specialist – For assistance with behavior, emotional issues, or ADHD.

Will the neurofibromas continue to multiply?

Yes, neurofibromas tend to increase over time, particularly during puberty and adulthood. Most are benign and do not cause harm, but some can grow and cause discomfort or cosmetic concerns. While they cannot be fully stopped, doctors monitor them closely for any complications.

Are the learning issues linked to NF1?

Yes, about 50 to 60 % of children with NF1 experience learning delays, attention issues (like ADHD - attention-deficit/hyperactivity disorder), and motor coordination problems.

School accommodations to request: Consider requesting an IEP (individualized education plan) or a 504 Plan for:

  1. Reading support.
  2. Occupational therapy for motor skills.
  3. Extra time for tests.
  4. Preferential seating.
  5. Behavioral support if needed.

I hope this helps.

Kindly revert so I can assist you further.

Thank you.
