Why is my period irregular with prolonged light bleeding?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am writing about irregular periods. I missed my period a month ago. Around two weeks ago, I noticed a brownish discharge that lasted for about a week. After that, the bleeding changed to dark red, then bright red, and my period is still ongoing. The flow has been light to medium, not heavy. My periods have always been regular before this. Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I read your query and can understand your concern.

Irregular bleeding like this can be very unsettling, especially if your periods are usually regular. From what you described, skipping your periods, followed by prolonged brown spotting, then irregular red bleeding lasting over two weeks with varying flow, here are some common possible causes.

  1. Hormonal imbalance: Stress, weight changes, thyroid issues, or PCOS (polycystic ovary syndrome) can disrupt your cycle and cause irregular or prolonged bleeding.

  2. Ovulation-related spotting: Sometimes, spotting happens around ovulation or delayed ovulation, causing irregular timing and variable bleeding colors.

  3. Endometrial causes: The uterine lining might shed irregularly due to hormonal issues or other factors, leading to prolonged spotting or bleeding.

  4. Infections or inflammation: Cervical or uterine infections can cause irregular bleeding and unusual discharge.

  5. Other causes: Less commonly, uterine polyps (small, soft growths that develop on the inner lining of the uterus), fibroids (non-cancerous (benign) muscle tumors), or bleeding disorders may cause prolonged bleeding.

What you should do next:

See your gynecologist for a thorough evaluation soon, especially since the bleeding has lasted weeks and is ongoing. They may recommend:

  1. Pelvic exam.

  2. Ultrasound of the uterus and ovaries.

  3. Blood tests for hormone levels (thyroid, prolactin, FSH (follicle-stimulating hormone), LH or luteinizing hormone).

  4. Possibly a Pap smear (Papanicolaou test is a screening test that checks for abnormal cells on the cervix to help detect cervical cancer or precancerous changes early) or infection tests.

If the bleeding becomes heavy, or if you experience dizziness, severe pain, or pass large clots, seek urgent medical care.

Meanwhile:

  1. Track the bleeding amount, color, and any symptoms (pain, fever, odor).

  2. Avoid heavy physical activity.

  3. Use sanitary pads (not tampons) to monitor bleeding.

  4. Stay hydrated and eat well.

I hope this helps.

Please revert in case of further queries.

Thank you.
