Is it safe to mix Hydroquinone with Azelaic acid?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello, doctor,

I want to ask about using 20 % Azelaic acid and 4 % Hydroquinone together for hyperpigmentation around the mouth. I have been using Azelaic acid for a long time, and it did not help with the hyperpigmentation around my mouth, but it has been great for my skin. How can I add Hydroquinone to my routine if it is fine to use both?

Please suggest.

Hello,

Welcome to icliniq.com.

I understand your concern.

Here are the answers to your questions:

Can I use Azelaic acid 20 % and Hydroquinone 4 % together for hyperpigmentation around the mouth?

Yes, they can be used together, but with caution. Azelaic acid 20 % is known for its gentle brightening and anti-inflammatory properties. Hydroquinone four percent is a more potent depigmenting agent, effective for stubborn hyperpigmentation (like melasma, post-inflammation hyperpigmentation (PIH) around the mouth). Because Hydroquinone is stronger and can irritate, combining both should be done gradually and with close monitoring for irritation or dryness.

How can I safely add Hydroquinone to my routine?

Here is a safe way to introduce Hydroquinone four percent while using Azelaic acid:

Start slowly. Begin with Hydroquinone four percent only at night, two to three times a week for the first week or two. Monitor how your skin reacts (redness, peeling, or burning).

Application order:

  1. PM (post meridiem)/night-time routine: Cleanser, toner (if using), apply Azelaic acid first if you want to layer them (as it is a milder exfoliant and brightener). Wait 10 to 15 minutes for Azelaic acid to absorb. Then apply Hydroquinone 4 % only on the affected areas around the mouth (not all over the face). Follow with a moisturizer.

  2. Alternate days: If irritation occurs, use them on alternate nights (for example: Hydroquinone thrice a week, Azelaic acid on other nights).

  3. Morning routine: Do not use Hydroquinone in the morning. Use Azelaic acid (if desired) with sunscreen having SPF (sun protection factor) 30+ every morning. Sunscreen is critical because Hydroquinone and Azelaic acid make skin more sun-sensitive.

I hope you find this information helpful.

Thank you.
