Hello doctor,

I am 29 years unmarried man. My penis upper skin was tight so I did circumcision 50 days before. Before circumcision, I was not having any problem but after circumcision, I have lost the sensation and even if I watch porn, still sperm is not coming out. Could you please advice?

I can understand your concern about your symptoms. It is post-surgical, I can give some time. Other reasons are such as, stress, depression, fear of failure, low self-esteem (chronic masturbation), homosexuality, religious constraints, long term illness, a negative attitude towards your partner, hormonal issue. and long term drug usage. I do not have any of such reasons what you have mentioned in your answer. While I am watching porn I am in cool mind and stress-free but still, I am not getting my sperm out. I tried three to four times to get sperm out after circumcision but still not getting that sensation what I used to get before circumcision. So, I have stopped trying. If this continues, then obviously I will get stress. I request you to please advice.

So even when you watch porn and masturbate you are not able to ejaculate sperm. Please confirm whether you get an erection. If you do not get an erection, then we need to think of hormonal imbalance of recent origin. If this issue had happened for you earlier on and off then we can think of obstruction and do the investigation. If you have already done investigation please share. If you do not get an erection at all, post-surgical need to investigate and review the surgeon who operated on you. If it is a urologist it will be better. Did you find any reason for tightness of your skin, were you having it from birth or recently you have noticed it? If you have recently noticed and had procedure then did you find any reason for what you had tightness now, anything to do with diabetes (if you had an investigation done, please share).

I do not know the exact meaning of erection but while I watch porn my penis gets hard and enlarge but ejaculation is not happening how it was before circumcision. Before surgery within ten minutes of watching porn, I can get sperm out but now it has fully stopped. Earlier, I was not having any issue with ejaculation. After surgery only it is there. I am not getting that much sensation to make the semen out. I consulted the urology doctor who did my surgery. He told nothing wrong in surgery and if I do not have an ejaculation, he told to me to consult an andrologist. I fade up. The foreskin was tight from my birth. I was not able to do full masturbation since my upper skin was not coming fully down. The doctor told after marriage you will get pain while having sex so I did.
