What causes painless penile blisters with fluid discharge?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I started experiencing itching on my penis about a week ago. Now, four or five blisters have appeared on the head of the penis. They do not cause any pain, but fluid comes out of them. Two of the blisters are located very close to the opening of the urethra (urine hole). I can send you the picture. I am using Trubax cream (Terbinafine Hydrochloride) and Alergocit 5 mg currently.

Please help.

Hello,

Welcome to icliniq.com.

I read your query and understood your concern.

Thank you for sharing your symptoms. Itching with fluid-filled blisters on the penis, especially near the urethral opening, needs careful evaluation.

Possible causes are as follows:

  1. Herpes genitalis (genital herpes): Typically causes clusters of small, fluid-filled blisters on the penis or around the genital area. Itching and blistering without much pain can occur, but often there is some discomfort or burning. Blisters near the urethral opening are common in herpes.

  2. Fungal infection or candidiasis: Terbinafine cream (Trubax) is for fungal infections; if this was prescribed, it might be to treat a suspected fungal rash. However, fungal infections usually cause redness, scaling, and itching rather than blisters.

  3. Contact dermatitis or allergic reaction: Could cause itching and rash, but blisters are less common.

  4. Other infections: Less commonly, syphilis or other sexually transmitted infections can cause ulcers or sores, but usually not fluid-filled blisters.

I suggest you follow the instructions below:

  1. Stop using any creams without a confirmed diagnosis, especially if your symptoms worsen or do not improve.

  2. Avoid sexual contact to prevent spreading any infection.

  3. Keep the area clean and dry.

  4. Avoid scratching to reduce the risk of secondary infection.

I suggest you consult a healthcare provider urgently, preferably a dermatologist, urologist, or a sexual health clinic. They can examine you and possibly take swabs or blood tests to confirm herpes or other infections. Early diagnosis is important for effective treatment, especially for herpes, which requires antiviral medications.

I hope this helps.

Kindly follow up if you have more doubts.

Thank you.
