My sugar levels are high despite taking insulin. Why?

I am a known diabetic for the past 15 years. 20 days ago, my HbA1c was 9.83. I am 54 years old and my weight is 87 kg. My medications are Metformin 500 mg, Hcl 1500 mg, Istavel 100 mg and Glaritus Insulin 30 units. Even then the sugar levels are 246, 265 and 277 after every three hours today. My creatinine, SGOT, SGPT, T3 and TSH are normal.

What causes bad taste and constipation?
Hello doctor,
Two weeks ago, I started Wellbutrin SR. I started with a dose of 100 mg in the first week. In the second week, I went up to 200 mg following doctor's orders. The doctor also prescribed Trazodone 100 mg at night when needed for sleep. I stopped taking Trazodone because I felt drained and exhausted all day. The next day, I was also taking Wellbutrin in the morning. Once I stopped the Trazodone, I immediately felt a difference. I was not exhausted and I was not irritable. It has been five days since I started experiencing a bad taste in my mouth and overall I am not feeling good. I have also been taking Phentermine 37.5 mg for weight loss for the past two weeks. I am also on a birth control pill. Could this be the cause of it? I have a bad taste in my mouth, constipation, headaches, and a stomachache. In the past, I have taken Lexapro for depression. I have gained weight of 20 pounds in three months, have lost the sexual drive, and I have exhaustion and I am irritable. What can be done?

Can a cyst in the right breast cause pain and discharge?
Hi doctor,
For the past two months I have had ongoing right breast pain. It first began as sharp stabbing pain that would last only a few seconds. Now I am getting some nipple discharge and burning sensation. When I spoke with my doctor about it, she said it might happen sometimes. I had some blood work done for my hormones which confirmed my testosterone was high. I have attached my hormone test report for your reference. I am taking now Estradiol 2 mg, Progesterone 80 mg and Testosterone 2 mg applying 1 cc to inner thigh daily. My previous history includes complex ovarian cyst, hypertrophy of uterus, dysmenorrhea, menorrhagia and hair loss. Should I get further testing from another doctor? Last year it was confirmed on ultrasound that I have two cysts in my right breast? Could this be the cause for my issues? Sometimes, applying a heat pad to my right breast helps. At times it will swell up so large and I feel like it is going to burst. This is concerning me. Please help.

What can be done to manage migraine which is getting worse?
Hello doctor,
I am a 22 year old female. I have always suffered from migraines. Recently, it seems like it has been getting worse. Last year, I gave birth to my son vaginally. Now, when I lay on my abdomen or even turn my head, I notice a heartbeat sound in my ear. My nurse practitioner thought it was coming from my neck and sent me for physiotherapy. And, she checked my ears, but there was no wax. Do I need physiotherapy? Should I get a referral and maybe go to a specialist? Should I be worried about a possible hearing loss?

Could neck pain, stiffness, hand numbness indicate syrinx?
Hello doctor,
I am having significant neck pain that goes out into my shoulders a bit. Pain is accompanied by severe stiffness. Also, my hands have been falling asleep about seven or eight times a week (roughly) and even ache a bit. I thought I might be dealing with a pinched nerve, but all my symptoms are fairly bilaterally equal. This last week I was put through an MRI for a lipoma in my left shoulder and neck area and they found that I also had a lower cervical syrinx at C6 about 3 mm. Could this be causing the pain and numbness that I have been dealing with? Or is the syrinx something that should be removed? I am awaiting an appointment with a neurosurgeon but I thought I would reach out in the meantime. I appreciate any information that you give me.
