Child spoke two words until 18 months, then stopped. Advice?

Hello doctor,

I am a mother of a 26-month-old boy. He usually developed until around 18 months then began falling behind. He had maybe two words by 18 months, and now he has none at all. We strongly suspect he has autism (many red flags present, MCHAT score of 13) and are working towards getting him a diagnosis and treatment but we live in a developing nation where professionals in this field are scarce, and waiting lists are long.

My question pertains to receptive language. Up to around 18 months, I felt sure he understood most of what was said to him. At present, I do not think he understands language at all. I have heard of autistic regression where children lose words, but do they lose receptive language too? Is this something you have seen?

Does hearing damaged get worse after hearing loud noise?

Hello doctor,

I have a question about hearing. Firstly, I am aware that hearing loss takes place when the tiny hair cells within the inner ear are damaged. My question is, once a particular hair has been damaged, will the same sound level that previously damaged it, hurt it again? For example, if you listened to a sound at 90 dB for nine hours and your hearing was damaged, would doing the same thing another day further damage your hearing? Or would it not change anything, as your ears would not be registering the same level of sound as the previous time?

I have trouble in uttering certain words. How do I overcome this?

Hello doctor,

I have a difficulty in pronouncing words that start with a vowel, for example, 'experience' or 'academic'. Whether the word is in the starting or the middle of the sentence, there is always a difficulty. It has been around two and a half years since the problem began. I have to take a momentary pause and thrust it out forcefully. There is an unusual feeling in my chest too when I force myself to speak such words. The special aspect of this problem is that there is no problem when I practice saying these words in my room. But, when I speak to others, it gets revived. I need you to suggest some speech drills, breathing exercise, or special precautions, so as to help me get over this probelm.
